I am using a RCBS neck turner with auto feed. It cuts a the outside of the neck but also has a pilot/inside reamer. When finished, there is a small ridge on the inside of the case at the neck and shoulder junction.
This will get me started. Those doughnuts didn't matter before as I was not seating bullets deep enough. The issue developed as I was trying some heavey cast gas checked bullets that are a hair bigger. They definitely held up on the doughnut.
check the depth of the "inside" pilot/neck reamer. I use the RCBS system and make sure the pilot reamer extends far enough in the neck not to leave the doughnut. Some necks are too long to allow that; however, most are not.
