It's a one piece ejector hand assembly made by Wild West guns. If you look at the factory ejector it's a two piece design with a piece of spring steel crimped into the ejector hand and I guess there have been a few come apart. They aren't too expensive and I believe you can still get them from either brownells or midway and all you have to do is remove the bolt to change it.
